Reverb PCB board ID

received this reverb PCB in a grab bag. Any one recognize the board and know the name or maker. I have the belton brick in my unused old parts and would like to use it with this PCB. Maybe someone has a BOM which would be great. i know this is a long shot but.... Thanks Hefty

I have one of those too. It was made by n8b5150

I don't remember where I found the docs for it, but I think it was just the datasheet circuit for the belt on brick

After building it, I found that R14 needs to be changed to 10 ohm. I used board mounted jacks and pot, but would suggest only board mounting either the jacks or the pot as it can be difficult to mount it in the case with both being board mounted.

Built it but ran into an issue with a bad opamp and never swapped it out. Gotta do that tho, now that I think about it.
