http://moosapotamus.net/ has one. Don't know much about it but I had a build doc for one from there.

edit: its for the MN3007 I just noticed. Might work for you.

It did not, it used SAD1024s and later a 3008 and 3009 which is probably what you're thinking of.

The A/DA is the only commercial design I can think of that used the chip, it wasn't used much, couple of rack mounts.
