Tone Monger Ross phaser clone

I bought a board from someone, I don't remember who. I need a picture of the board, it's an OSHPark tabbed board. I need to troubleshoot it. I think it's a bad set of LM chips, but I want to check the rest of the board. Anyone know who that is?
