This one has 6 jacks  The circuit is JMK's Paralyzer. I connected the send and return jack switched tip contacts so that if the loop is left empty it functions as a clean blend. This is a pretty cool circuit and I've only had about 10 minutes to play with it but the possibilities are endless.

Some of the more talented builders would have found a way to put a stomp switch in there 
I'm not finished with the enclosure yet, I'll have to file some of the lip off of the bottom plate for it to fit and I'm planning on doing a simple graphic for the top so you know what's actually going on.

I found them on ebay from china. I saw them on th thread here with a belton reverb in a 1590a. I think small bear has them too. Probably better quality. The nut has a lot of play as is typical for Chinese parts.
