1) Can ground be the top of the pot or does it need to go to the ground pad on the board?

3) Does it make any difference if R12 or R13 is used? R13 is near the edge of the board and might be easier to pull out.

1) Technically yes, if your enclosure is grounded, but I'd recommend grounding at another point.

2)Follow the schematic with the layout. Sorry I left this with not much info because you'll learn more if you figure it out yourself  ;)
