Transformer isolated A/B/Y layout

I just got the transformers for this but I'm not sure how to read the pins on the transformer from the schematic. Can anyone take a look at this?

Primary side (left) in the schematic: pin 1 is signal, pin 3 ground.
Secondary side: pin 4 to the jack tip, pin 6 to sleeve.
