Title: Need a PCB ID and possible build doc??

Looking for a board ID for the following PCB. A link to the build doc would be lovely as well...  ;)

I am going to venture and say that's a 2-in-1 with a Cornish SS2 & G2. And that it is a Black Horse Effects (electricb) pcbs.

Now that the PCB has been identified (many thanks to micromegas) can anyone help with the following?

Has anyone successfully used subs for the LM1458 and the 1N60Ps in this circuit? I have plenty of BAT-type diodes and also many, many dual op amps (KA4558 is a close sub)

Opinions??

4558 will work for the 1458, or any BJT input dual op amp

There's some Darkside (G2) diode discussion in another thread. I used AA113 diodes and like them.
