There's your issue, dual opamp in a single opamp circuit... but i've not seen a resistor with that colour code before... red, black, black, black, black, black, white, white, black.
There are only 6 leads on that op amp. :/

You're gonna have to tack solder that thing. It won't stay in place.
Having just watched me loose sprite through my nose at this, my wife has graciously pointed out that we are all dorks.
so red box with black knobs for this one ... I guess it asked for it.
I've heard of dead bug circuits, but live bugs??? That's a new one!
